Title: Patient Support Teams

Reports To: Volunteer Coordinator

Description:

Not everyone has time to volunteer on a regular schedule. That’s why starting or joining a Patient Support Team may be the opportunity you’ve been looking for! Patient Support Teams are groups of two or more trained Patient Support Volunteers who are contacted when we have an urgent need nearby. One or more of your group members can step up and support a patient when no one else can! Responsibilities may include but are not limited to:

  • Conversing with patients
  • Provide respite for family or caregivers
  • Light housework
  • Reading, watching TV, or playing games with patients
  • Documenting visits in an online medical record

Location: Homes and care facilities in the Stockton or Sacramento area

Qualifications:

  • Members must all be individuals, supportive of the hospice concept
  • Comfortability working independently while also communicating with a team
  • Willingness to work with patients and family regardless of gender, religion, race, sexual preference or economic status
  • Able to respect patient confidentiality – demonstrate understanding of HIPPA
  • Able to maintain appropriate and professional boundaries with patients
  • Able to listen and practice patience
  • Basic computer skills
